RETIREMENT CHECKLIST
As your retirement approaches or arrives, you should examine these items.
- Filing Your Application for Retirement
- You’ve carefully planned your retirement, received your benefits estimate and are ready to take the big step.
- Before you can start receiving a pension, you’ll need to complete & submit your Application for Service Retirement (RS6037).

- Beneficiaries - Who are they?
- NYS retirement (if you have selected an appropriate plan)... NYS Retirement Website
- City of Syracuse (incorrectly called the PBA insurance) $25,000 free life insurance. At age 60, the coverage reduces to $10,000.
- Syracuse Police Fraternal Association
- Deferred Income Plans
- Social Security Income
- Health Insurance - What's Covered?
- Usually everything that was covered while you were active on the PD
- You must make co-pays per visit (usually ending once Medicare is in effect)
- At age 65 you and your spouse will be transfered to the current City Medicare PPO agency, for health insurance coverage
- Your surviving spouse may continue health insurance coverage (as stated above), for a monthly premium, at their expense.

- Update or obtaining a gun permit
- Obtaining direct deposit of your NYS Retirement Plan
- Continuing your City of Syracuse Dental Plan (for 18 months), at your cost.
- SSI & Medicare programs
- Loan payments that are currently paid via payroll
